COVID-19 Forms & appointment arrival instructions


To minimize contact while in our office, please read, sign, and submit BOTH required forms below as instructed. No printing necessary.

Your appointment is automatically canceled if we do not receive both forms in a timely manner.

Please discuss with us ahead of time if any of your answers is “yes”. We appreciate your honesty and willingness to keep yourself and others safe.


Arrival instructions

We kindly ask that ONLY the scheduled patient come to his/her appointment. If the scheduled patient is a minor, he/she may be accompanied by one parent/legal guardian. One translator is allowed if needed.

  • All patients are required to wear a mask before entering our facility.

  • Mask must cover nose and mouth AT ALL TIMES, including lobby and private treatment rooms.

  • Masks with valves and neck gaiters are NOT acceptable for entry.

    (Research shows they do not protect others from wearer’s potentially infectious droplets. Masks with 3 layers of cloth are preferred)

  • No patients are allowed to wait in the lobby (unless discussed ahead of time for exception).

  • Please call our front desk at (562) 860-8609 to let us know you’ve arrived, and wait in your vehicle for further instructions.

    (If we do not pick up right away, please be patient as we are assisting patients or in process of sanitizing your room. We will call you right back when we are ready for you.)

  • We will direct you to fill out and submit our health screener and/or other necessary forms.

  • Once we receive your submitted forms, we will give you a call when we are ready for you to come inside our facility.

  • All rooms are sterilized with surgical-grade UV light when a patient leaves and requires time. We appreciate your patience and understanding of this safety measure.

  • Your temperature will be taken, and hand sanitizer is required upon entry.

  • We strongly recommended washing your hands with soap for 20 seconds whenever possible.
